72 Hour Vitals: High Monitoring – High monitoring is an extra set of
vitals that are to be done at 12:00 PM within the first 72 hours of a client’s
intake. It is the Charge Staff responsibility to ensure that this is
completed.
Phone Procedure – We are allowing clients to use the
phone between 5:30 & 9:00 PM, unless there is programming. Clients
will sign the phone out from a designated office. Brian created a form
that is in the Phone Binder, located in the staff office, to track the number
of phone calls a client makes along with the time out and time in of the
phone. Clients will be responsible to return the phone to staff within
the 10-12 minute time period allotted. Clients who abuse phone privileges
or are late need to be reported to their Coordinator. This form stays in
the Phone Binder until the following Sunday, when the phone calls reset.
When a new client comes in that is not on the phone roster, there is a location
on the form to hand write them in.
Client Property Inventory Procedure –
Jim has organized a
client property inventory system in the Coordinator office closet. When
we take possession of any client property, we need to be put it in a client
property bag and list it on the inventory card along with the date it was
taken. When a client discharges or sends property home, the client needs
to sign the inventory card so that we have record of the property being
released.
Appointment Procedure – Clients will now be scheduling their
own appointments and setting up their own transportation via a med cab.
They will notify staff of the date, time, and location of the appointment so
that it can be entered into the database and we can generate the appropriate
paperwork for them to sign and have the clinic fill out. Appointments
still need approval, and only appointments that are critical to a client’s
treatment plan will be approved. We will still be searching and drug
testing all clients upon returning from appointments.
New Dominion Form – There is a new Dominion form that has
been added to the intake paperwork. Please fill out both Dominion forms
and put them in Chad’s box when they are complete.
